We routinely call to confirm patient appointments 1-2 days in advance of your appointment. If you do not hear from us and believe you have an appointment, please notify us at 859. 745. 6480.

 

New Patients

Welcome to Central Kentucky Cardiology. The following will help you in setting up your first appointment.

Once we have made the appointment, you will need to visit the forms section of our website, print and complete the forms listed below, and have them with you on the day of your scheduled visit. If you have any questions, please contact the office that you are scheduled to visit.

For your first visit with us, it is important that you bring the following information with you:

A Any information from your medical record, that pertinent to your cardiac condition.
B All medications you are currently taking, in their original containers.
C Copies of the reports for the following tests, if you had them within the last five years: EKG, Holter Monitor, Event Recorder, Stress Echocardiogram, Dobutamine Echocardiogram, Transesophageal Echocardiogram, Angioplasty, Catheterization, Pulmonary Function Tests and Electrophysiology Studies.
D So that we can communicate the results of your visit with your primary-care physician, it is important that you bring your physicians name, address and phone number to your visit, as well as any written referral information. Please bring your health insurance information with you to the visit. Your insurance carrier may require an authorization for your first visit. Please insure that you have the appropriate referral authorization with you at the time you come in for your appointment.
E Payment for services not covered by your insurance plan or any insurance co-payments are expected at the time of service. If you have any questions or concerns regarding billing issues, please contact either your insurance carrier or our billing office at (555) 555-5555.

If this is your first visit to Central Kentucky Cardiology, you will need to arrive 15 minutes prior to your scheduled appointment time for registration.
